Case #1

Age 60 Male, nonsmoker, 5 ft '8 inches, 220 lbs with a history of poorly controlled hypertension for 17 years.  Taking Lisinopril 20 mg, Felodipine 10 mg & Crestor 10 mg.  Recent surgery for aortic aneurysm. 

Declined by USFL but issued table 4 by our carrier the same year.

Case #2

Male orthopedic surgeon, age 57 non-smoker who was declined due to 'poorly controlled' diabetes. A1C was 11.3. Single single vessel angioplasty in 1998. Follow up stress test in 2002 was 'normal'.

Client was 5 ft '7in, 143 lbs and was first diagnosed with diabetes at age 38. Took humulin and altace. Family history of Diabetes and hypertension.

USFL declined coverage. Our carrier offered T-6 for term. Agent was paid on a $19,000 premium.

Case #3

Male age 55 with a history of coronary artery disease. Underwent bypass surgery in 1998. Family history is unfavorable, father died of a heart attack at age 45. Follow-up was poor. Stress cardiolite EKG study was performed in November of 2001, but follow-up stress cardiolite study, 18 months later was never completed. Blood lipids were not followed after November 2001. Case was submitted in 2004.

USFL declined coverage, but our carrier offered Table 5.

Case #4

Age 65 male non-smoker with a history of colon cancer treated with surgery and chemo. Full recovery in 1995. Melanoma diagnosed in the spring 2000 on scalp, level IV, 1.3 mm, no recurrence, regular follow-up.

Client was 5 ft 11 in, 215 lbs and treated hypertension for 20 years. Lisinopril 5mg/day and hctz 25mg/day.

USFL declined coverage, our carriers offered Standard non-smoker plus $10 a thousand for 4 years. Offer was subject to complete pathology reports and follow-up from colon and skin cancer history.
